North Perth Vaccination Facility Requirements
September 16, 2021 – In compliance with the Province of Ontario requirements, effective Wednesday, September 22, 2021, all patrons (this includes; players, coaches, trainers, volunteers and spectators) 12 years of age or older wishing to access any of the municipally operated recreation facilities and/or their meeting spaces in North Perth will need to show proof of being fully vaccinated (two doses plus 14 days) along with proof of ID (Health Card, Driver’s Licence, Birth Certificate, Citizenship Card, Permanent Resident Card, Passport, Indian Status Card/Indigenous Membership Card) upon entry.
The following will be exempted from these requirements;
- Individuals who cannot receive the vaccine due to medical exemptions will be permitted entry with a valid note from a Registered Physician or Nurse Practitioner only until recognized medical exemptions can be integrated as part of a digital vaccination certificate.
- Children under 12 who are currently unable to receive the vaccination due to age restrictions will also be exempted from these requirements.
- Individuals who are under 18 years of age, and who are entering the recreation facility used for sports and recreational fitness activities solely for the purpose of actively participating in an organized sport, in accordance with guidance published by the Ministry of Health on its website for the purposes of this provision.
To obtain a vaccination receipt, individuals can download or print their vaccine report from the provincial booking portal at covid19.ontariohealth.ca or by calling the Provincial Vaccine Booking Line at 1-833-943-3900. Individuals requiring assistance to print their vaccine report can contact the North Perth Public Library at 519-291-4621.
